Job Title: Senior Practitioner – Integrated Learning Disabilities

Pay Rate: £36 per hour
Job Description:
Liquid Personnel is currently recruiting a Senior Practitioner on behalf of its client’s Integrated Learning Disabilities (LD) Team based in Hackney.
This role involves leading and supporting high-quality, person-centred social work practice within the Integrated Learning Disabilities Team. The Senior Practitioner will work with adults with learning disabilities, particularly those transitioning into adulthood, promoting independence, safeguarding, and wellbeing through integrated, multi-disciplinary approaches.
What will your responsibilities be?
- Hold a complex caseload and provide supervision and guidance to junior staff.
- Support the development of care plans that reflect individual needs and aspirations.
- Work with individuals aged 18–25 to support their transition into adult services and independent living.
- Promote strengths-based and trauma-informed approaches.
- Work closely with health professionals, education providers, housing, and voluntary sector partners to deliver integrated support.
- Lead multi-agency meetings and contribute to strategic planning
